Welcome to our practice. On behalf of Southwest Gastroenterology, thank you for selecting our office for your gastrointestinal care. Your health is very important to us. To expedite your care in our office, on this page you'll find the information you need to get started, such as a map and directions to our office. To save you time, we've also included our new patient forms, which you can complete prior to visiting our office.
In addition, please bring the following items with you:
- Identification such as your driver's license
- Insurance card(s)
- Current medications or a list of them
- Medical records pertinent to your case from your primary or referring physician's office
- A form of payment (We accept cash, check, MasterCard, or Visa)

For your convenience, our patient forms are located below. Please complete your form online so we can expedite your first visit. Incomplete forms, missing or erroneous information, and missing signatures will result in a longer than necessary waiting period.

Insurance and Billing
We contract with many major health insurance companies and accept several Medicare and Medicaid plans. Please contact our office to verify acceptance of your insurance plan. It is the policy of this office to verify insurance eligibility and benefits. Payment for services is expected at the time of service. If a procedure is recommended or necessary, please bring in payment to allow for any deductibles, co-payments, or patient financial responsibility. If you have not already done so, we strongly recommend that you check your plan provisions before arriving.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you.
